A dermatologist emailed us a screenshot last month. A competitor two miles away had that green Google Screened badge sitting above every organic result, above the map pack, above everything. "They look official and we look like an afterthought," she wrote. "How do I get the checkmark?" It is one of the most common questions we get right now, and it deserves a straight answer instead of a sales pitch.
Here is the short version. The badge comes from a Google product called Local Services Ads. Some medical categories can get it today. Many cannot yet. And even for the practices that qualify, the badge is only worth having if the rest of your setup is ready to catch the patients it sends. Let us walk through all of it.
What the green badge actually is
The checkmark is not something you buy on its own or earn through good reviews. It is attached to Google Local Services Ads, a special ad format that appears at the very top of the search results, usually above the regular text ads and above the map. Each listing shows your name, your rating, your hours, and that green badge.
You will see it in two flavors. Google Screened is used for professional and licensed categories, and it means Google verified your license and ran a background check. Google Guaranteed is more common for home services and can come with a money back guarantee for the customer. For most medical categories that qualify, the badge you get is Google Screened, because what Google is vouching for is your credentials, not a service outcome. To a patient scrolling fast, the difference barely registers. They see a checkmark and they see that Google put you at the top on purpose.
The part nobody tells you: many specialties still cannot get it
This is where the honesty comes in. Local Services Ads rolled out to healthcare in stages. Google started with dental providers in early 2023, then added a batch of categories that same spring. As of now, the eligible healthcare categories include:
- Dentists and orthodontists
- Chiropractors
- Dermatologists
- Physical therapists
- Podiatrists
- Optometrists and ophthalmologists
- Allergists
- Dietitians
- Primary care physicians
- Weight loss centers
If you are on that list, good news, you can pursue the badge. If you are not, and a lot of practices are not, then no amount of effort gets you the checkmark right now. Mental health and therapy practices, most surgical specialties, cardiology, gastroenterology, OB GYN, med spas and many elective categories are not part of the Local Services Ads program as we write this. The list has grown over time and will likely keep growing, so the right move is to check your exact category directly in the Local Services Ads signup rather than assume. But do not let anyone tell you every practice can get the badge. That is simply not true today.
How the money works, because it is different
Local Services Ads do not work like normal Google Ads. Regular search ads charge you every time someone clicks, whether or not they ever contact you. Local Services Ads use a pay per lead model. You are only charged when a patient actually calls or messages you through the ad. A click that goes nowhere costs you nothing.
The price per lead swings widely by specialty and city, from the low tens of dollars in less competitive categories to well over a hundred dollars in crowded cosmetic and elective markets. You set a weekly budget, Google fills it with leads, and you can dispute obviously bad ones, like a wrong number, a spam call, or someone clearly outside your service. On paper that sounds great, and often it is. But it hides a trap we see constantly, and it is the same trap that wastes money on every kind of Google Ads for a medical practice: you pay for the lead whether or not you answer it.
The badge is not the hard part. Answering the phone is.
Here is the pattern we watch play out. A practice turns on Local Services Ads, gets the badge, and the calls start coming. Then the front desk is slammed at 11am, three calls stack up, two roll to voicemail, and nobody calls back until tomorrow. By then those patients booked somewhere else. The practice paid full price for every one of those leads and converted a fraction of them.
Speed is everything here, and the numbers back it up. We dug into this in our piece on how fast you should respond to a new patient lead, and the honest truth is that a lead answered in the first minute is worth many times one you get to an hour later. A pay per lead ad only makes sense if there is a reliable system on the other end of it. That is exactly why we pair paid channels with a way to make sure no call is missed, including our AI receptionist, so a patient who calls at a busy moment or after hours still gets answered and booked instead of dumped into voicemail.
If your specialty cannot get the badge, you have not lost anything real
This is the reframe that matters. The green checkmark is not magic. It is a trust signal. And trust is something you can build several ways, most of which do not depend on Google adding your category to a program.
Think about what the badge is really doing for that competitor: it tells a nervous patient "this one is legit, other people trust them." You can send that exact same signal without it.
- Reviews do most of the work. Around 84 percent of patients read online reviews before choosing a new provider, and most trust them nearly as much as a recommendation from a friend, according to review research summarized by RepuGen. A steady stream of recent, genuine reviews is a trust badge you fully control.
- The map pack is prime real estate. Appearing in the local three pack for searches like "dermatologist near me" captures a large share of the clicks for local medical searches. A complete, active Google Business Profile is how you earn that spot, and it is free.
- Responding to reviews raises trust further. Survey data shows a large share of patients trust a provider more when they see the practice replying to reviews. That is a signal you send with a few minutes a week.
- Local SEO puts you where patients look. Ranking organically for your services and your town keeps bringing patients long after an ad budget runs out.
None of that requires a green checkmark. It requires showing up consistently in the places patients already look. That is the heart of healthcare SEO, and it works for every specialty, eligible or not.
So should you chase the badge?
If your category is eligible, your service area is competitive, and your front desk can reliably answer and book new calls, Local Services Ads are worth testing. The badge plus the top of page placement plus the pay per lead model is a strong combination when the follow up is tight. Start with a modest weekly budget, dispute bad leads, and measure how many of those calls actually turn into booked patients, not just how many calls came in.
If your category is not eligible, do not spend another minute frustrated about it. Put that energy into reviews, your Google Business Profile, your local rankings, and a website that loads fast and makes booking effortless. You will build the same trust the badge signals, and you will own it instead of renting it one lead at a time.
How EtherealMinds approaches this
When a practice comes to us asking about the badge, we do not start with the ad. We start with two questions. Can your category even get it, and if a flood of new calls arrived tomorrow, would you catch them? Only then does it make sense to talk about turning on any paid channel. We build the full picture around it, a fast website, strong reviews and local SEO so patients find you organically too, and a front desk system so no lead is wasted. Ads should be the accelerator on top of a machine that already converts, not a patch over one that leaks. That is what a real patient acquisition system is built to do.
The honest bottom line
Can your medical practice get the Google Guaranteed green badge? If you are a dentist, chiropractor, dermatologist, physical therapist, podiatrist, optometrist, allergist, dietitian, primary care doctor or a few other listed categories, yes, through Local Services Ads, and it can pay off well if your phones get answered. If you are outside that list, not yet, and that is genuinely okay. The checkmark is a shortcut to trust, not the only road to it. Reviews, a strong profile, local rankings and a practice that answers fast will earn a patient's confidence with or without the green mark next to your name.
