Picture the search everyone in healthcare is fighting over. Someone types "dermatologist near me" or "knee pain doctor" into Google. The results load. At the very top sit two or three ads with a small "Sponsored" tag. Below them, the Google map with the three nearby practices. Below that, the regular blue links. So where does the finger go?

The instinct of most practice owners is that the top wins, so they either pour everything into ads or give up on ranking. Both moves are built on a guess. And the data on how people actually behave on that page says the guess is usually wrong.

~5% Of people say they trust paid ads more than organic search results. Close to half trust the organic listings more.

The short answer: most patients skip the ads

For as long as search marketers have measured it, the majority of clicks have gone to the free organic listings, not the paid ads sitting above them. The exact split moves around by study and by how commercial the search is, but the pattern holds: a large share of people, in some analyses close to 70 to 80 percent, scroll right past the ads and click something Google ranked rather than something a business bought.

Click through rate by position tells the same story. Data from First Page Sage, updated for 2026, shows the number one organic result pulling roughly 39 percent of clicks, with the first handful of organic listings soaking up most of what is left. The top paid ad, by comparison, earns a much thinner slice. When a patient wants a real answer, the organic results win the click far more often than the ad above them.

In healthcare that lean is even stronger, because the stakes are personal. Choosing who examines your skin or operates on your knee is not the same as buying a phone case. People slow down, they read, and they lean toward the option that looks earned instead of the one that looks paid for.

Watch yourself do it

Next time you Google something you actually care about, notice your own eyes. They flick down past the "Sponsored" block almost automatically, hunting for the first result that feels like a real answer. Your patients do the exact same thing. Nobody taught them to skip the ads. They just learned, the same way we all learned to ignore the banner across the top of a website. The ad is still there. The attention is a few inches lower.

Why patients skip the ads

Two forces are at work, and understanding them tells you what to do with your budget.

Habit. After twenty years of ads dressed up to look like results, people have built a reflex to tune out the "Sponsored" label. Marketers call it ad blindness. The eye slides past the paid block and lands on the first thing that reads as organic. It is not a decision so much as a trained skip.

Trust. This is the bigger one for healthcare. When someone is choosing a doctor, a listing Google ranked on merit sends a signal of credibility that a bought placement cannot match. Consumer surveys reported by Search Engine Land and others have found only around 5 percent of people say they trust paid ads more than organic results, while close to half trust the organic listings more and the rest trust them about equally. Some patients even wonder whether the result at the very top is the best option or just the practice that paid the most. That doubt is a tax on every ad you run.

~39% Share of clicks the number one organic result earns, far more than the top paid ad (First Page Sage, 2026).

None of this means ads are useless. It means ads are working uphill against a trust gradient, and you should know that before you decide they are the whole plan. If you have ever wondered why Google ads feel so expensive for medical practices, part of the answer is that you are paying premium prices for the slot most people skip.

The twist: paid clicks are gaining ground

Here is where we have to be honest instead of comfortable, because the page is changing under everyone's feet. The old "organic wins by a landslide" numbers are still directionally true, but the gap is narrowing fast.

A 2026 analysis covered by Search Engine Land found that between January 2025 and January 2026, classic organic listings lost anywhere from 11 to 23 percentage points of click share across major categories, while text ads picked up 7 to 13 points in every case. In some verticals the paid share of clicks roughly doubled. Two things are pushing this: Google keeps giving ads more real estate at the top, and AI Overviews, the AI answer that now appears above everything, are pulling clicks away from the traditional links. One September 2025 study found organic click through rates fell 61 percent on searches where an AI Overview showed up, and paid click through fell too.

So the story is not "ads are dead, just do SEO." The honest version is messier. Organic still earns the majority of clicks and almost all of the trust. But ignoring paid search means handing a growing slice of ready to book patients to whoever did buy the ad. And ignoring organic means being invisible in the spot most people still click first. We laid out the money side of this tradeoff in SEO versus Google ads for medical practices, and the conclusion there matches the click data here: it is not a versus at all.

What this means for your practice

Stop thinking of ads and organic as rivals fighting for one budget. They do different jobs, and a healthy practice uses both on purpose.

Ads are the down payment. They buy you the top of the page today, before you have earned a ranking. That makes them the fastest tool for a brand new practice, a new provider with an empty schedule, or a new service nobody knows you offer yet. Run them tight, with negative keywords so you are not paying for job seekers and freebie hunters, a landing page built to convert, and call tracking so you know which clicks actually became patients. Done loosely, ads are a leak.

Organic search is the equity. Ranking in the map pack and the regular results earns the clicks most patients prefer, carries the trust ads cannot buy, and keeps working after you stop paying. This is the slow compounding side of the house, and it starts with the basics: a fully built Google Business Profile, steady reviews, and real local SEO so you actually show up when someone nearby searches for what you do. If you are not showing up on Google at all, no ad budget will paper over that for long.

The real prize is owning the whole top of the page. When a patient searches your specialty, the strongest position is not the ad or the organic listing. It is all of it at once: your ad at the top, your practice in the map pack, and your website in the organic results. You take up the screen, you look established, and whichever way that patient's finger drifts, it lands on you. That is the goal, and it is very hard to fake.

How EtherealMinds handles both

The reason most practices end up guessing is that ads and SEO usually live in two different places, or in nobody's hands at all. An agency runs the ads and cares only about clicks. The website sits frozen and slowly falls out of the rankings. Nobody connects the ad spend to booked patients, so the whole thing runs on hope.

We build it as one patient acquisition system instead. The ads capture the ready to book searchers today. The SEO and local search work compounds underneath so you climb into the map pack and the organic results and stop renting every click. And it all points at a website built to turn a visit into a booking, with tracking that ties real patients back to the source, so you finally know what is working instead of arguing about it. Ad, map pack, organic listing, the whole top of the page, working together.

So, do patients click Google ads or organic results? Today, most still click organic, and they trust it more, but paid is climbing and AI answers are reshuffling the deck. The practices that win are not the ones betting everything on one square. They are the ones showing up everywhere the patient might look, so the answer to "where do they click" is simply: you.
