A patient searching for a doctor near me on a phone with a map, the local search moment where the Google map pack still decides who gets the call
When a patient types doctor near me, Google now serves the map and local listings, not an AI answer. That is the search that books the appointment. Photo via Pexels.

A physical therapy owner called us in a panic over the summer. He had read that Google answers most health questions with AI now, and he was convinced his practice was about to vanish from search. He wanted to scrap his whole local plan and pour everything into, in his words, whatever gets me into the robot. We asked him one question first: pull out your phone and search for a physical therapist near you. What do you see?

He paused for a second. Then he said, a map, three clinics, and a bunch of normal results. No AI thing at all.

Exactly. And that is the part the panic headlines leave out. Yes, Google uses AI Overviews all over health search now. But on the searches where a patient is actually looking for a provider to book, Google has pulled its AI answer out of the way. Understanding that split is the difference between spending your marketing budget where patients actually decide and chasing a trend that does not even show up where it counts.

What the data actually shows

Start with the fact that set off the alarms. AI Overviews really are everywhere in the research stage of health search. The analytics firm BrightEdge, which has tracked AI Overview coverage for years, found that by December 2025, 100 percent of treatment and procedure queries and 93 percent of symptom queries triggered an AI Overview, up from roughly 45 and 57 percent two years earlier. If a patient asks Google what causes plantar fasciitis or how a dental implant works, Google usually answers with AI right at the top.

Now the part nobody put in a headline. BrightEdge tracked the opposite curve for local and provider intent, the searches like dentist near me or dermatologist open today. That intent went from full AI Overview coverage at the end of 2023, down to 14 percent at the end of 2024, and all the way to 0 percent by December 2025. Google deliberately took its AI answer off the searches where someone is trying to pick an actual office nearby. On those, you still get the local map pack and the regular listings, with no AI summary competing for the click.

0% Share of local and provider intent searches that showed a Google AI Overview by December 2025, down from full coverage two years earlier, per BrightEdge. The near me searches that book patients are an AI free zone.

Put the two trends side by side and the story is clear. The research phase, the questions, the symptoms, the how does this work, is now AI territory. The booking phase, the near me and the open now, is still won the old fashioned way: by ranking locally and owning your listing. If you want to go deeper on that booking moment, we broke it down in how doctor near me searches work for medical practices.

Why Google backed its AI out of provider search

This was not an accident, and it tells you something useful. Getting a symptom explanation slightly fuzzy is low risk. Getting a real provider's name, address, hours, or specialty wrong is a different kind of problem, because patients act on it, and they tend to believe it.

The 2026 Patient Choice Report from rater8, a survey of nearly 1,000 adult patients across the United States published in June 2026, found that 66 percent of patients had already seen incorrect provider information from an AI tool, and yet 60 percent still trusted the AI summary without checking it further. A confident, wrong answer about a specific doctor is exactly the kind of thing that gets a search engine in trouble. Pulling AI Overviews off provider queries is Google protecting itself, and the side effect is that your local ranking still does the heavy lifting on the searches that matter most to your calendar.

But do not relax about AI either

Here is where we tell the other half of the truth, because it would be easy to walk away thinking AI does not matter for your practice. It does. It just matters earlier in the journey than you think.

When an AI Overview does appear, patients trust it more than anything else on the page. In that same rater8 report, when people researched doctors on Google, the AI Overview was the most trusted part of the results at 37 percent, well ahead of the organic blue links at 20 percent, the local map pack at 13 percent, and sponsored results at just 7 percent. And patients are going straight to AI chat tools too: among those who searched for a doctor in the past year, AI tools like ChatGPT and Claude influenced 36 percent of them, edging out Google search at 34 percent and even doctor recommendations at 32 percent. We dug into that trust shift in whether patients trust Google AI Overviews.

So the patient journey now has two distinct moments. In the research moment, a patient learns about their problem from AI, and the practices that get named and cited there plant the seed. In the booking moment, that same patient searches locally for someone to actually see, and the map pack decides who gets the call. Win only one of those and you lose patients in the gap. If AI never mentions your practice when it matters, you are fighting the problem we described in why ChatGPT recommends hospitals and not your practice.

The two front test

Run two searches as if you were a patient. First, ask ChatGPT or Google a research question in your specialty, the kind a worried patient types at night. Does your practice get named anywhere in the answer? Second, search your specialty plus near me from your phone. Are you in the top three of the map? If the answer to either is no, you have a specific, fixable gap, and it is costing you patients who never even saw your name.

What this means for where you spend

The takeaway is not panic and it is not complacency. It is focus. Two plays, run at the same time.

Win the local searches that book

This is still the biggest, most reliable source of new patients, and AI has not touched it. That means a complete, accurate, active Google Business Profile, a steady flow of recent reviews, correct hours and categories, and a website fast and local enough to rank in the map pack. If you are not showing up there, the cause is almost always fixable, and we laid out the usual culprits in why your medical practice is not showing up on Google. This is the core of our healthcare SEO work: getting you found and chosen on the searches that end in a booked appointment.

Become the practice the AI cites

For the research stage, the game is being clear, factual, and quotable. AI tools pull from pages that answer real questions plainly, with genuine expertise and trustworthy signals behind them. That means publishing honest, well structured answers to the questions your patients actually ask, so the AI names you instead of a competitor or a big hospital system. It is the same discipline as ranking in a world of zero click search, where being the cited source beats chasing a click that never comes.

Make sure a found patient can book

Both plays are wasted if the patient lands on a slow, confusing site and gives up. Whether they came from the map or from an AI answer, the last step is the same: a clear, fast, trustworthy page with an obvious way to book. That is why a website built to convert and book patients sits underneath everything else. It is the one asset that catches patients no matter which door they came through.

Where EtherealMinds fits

We build the whole picture, not one slice of it, because patients do not move through your funnel in a straight line anymore. When we set up a complete patient acquisition system, we treat local search and AI visibility as two halves of the same job. We get your Google Business Profile and local ranking strong enough to own the near me searches that book, and we build content and structure clear enough that AI tools quote you in the research stage. We do it as a healthcare only agency, so we know the lines you cannot cross and the claims you cannot make. The goal is simple: whether a patient asks the robot or taps the map, your practice is the name that comes up.

Our honest take

AI is reshaping search, but not in the all or nothing way the headlines suggest. The most important shift of 2026 is not that AI is swallowing local search. It is that Google drew a line: AI answers for learning, human ranking for booking. Practices that understand that line stop panicking and start doing the two things that actually work, owning their local presence and earning their spot in the AI answer.

That physical therapist did not need to pour everything into the robot. He needed to fix a Google Business Profile that had the wrong hours and had not seen a new review in eight months, and he needed a few pages that actually answered the questions his patients were asking. Within a few months he was back in the top three of the map for his town, and his name started showing up when we asked ChatGPT who to see for his specialty nearby. Nothing vanished. The searches were always there. He just had not shown up on them yet. There is a very good chance the same is true for you, and finding out takes one honest look.
