It had a Book Online link, a New Patients link, a tap to call button, and its address with a little map. Hers sat right below it as two thin lines of text and a headline. Same city, same service, and her ad looked like the budget option even though she was paying more.
She had not done anything wrong, exactly. She just never set up her extensions. And that one gap was quietly making her expensive ads look cheap and get skipped.
What extensions are, and why Google renamed them
Extensions are the extra pieces that attach to a plain text ad and make it bigger and more useful: links to specific pages, a phone number, your address, a list of services, short lines that call out what makes you different, even images. Google renamed them to assets back in 2022 and moved them into the assets area of the account, so newer screens say assets while most people still say extensions out of habit. They are the same thing. Do not let the wording throw you.

And Google factors your assets into ad rank, the formula that decides who shows up first and how much each click costs. So a well built ad does not just look better. This is not a small tweak. Google's own guidance, repeated across years of agency testing, puts the average click through rate lift from adding assets at roughly 10 to 15 percent. For a practice, the four that earn their keep almost every time are Book Online, New Patients, Services, and Insurance We Accept. Someone searching in a hurry sees Book Online and skips straight to your calendar. Someone cautious clicks Insurance first. You are meeting both without making either dig.
One caveat: a sitelink is only as good as the page behind it. If Book Online dumps them on a slow homepage where they have to hunt for the button, you wasted the click. 2. Callouts: the one liners that answer the silent question
Callouts are short phrases, not links, that sit under your ad and name what makes you easy to choose. Things like Same Day Appointments, Evening and Saturday Hours, In Network With Major Plans, Free Parking, Board Certified, or New Patients Welcome. Each one quietly answers a worry a patient has before they call. They are tiny, and they are one of the most underused assets in healthcare accounts.
3. Call asset: let them tap to call you
The call asset puts your phone number on the ad so a patient on their phone taps once and you are ringing. For a lot of practices, especially anyone with a busy front desk, calls still convert better than forms. But here is the honest catch that most guides skip: a tap to call button is only worth setting up if someone actually picks up. 4. Location asset: show your address and map

5. Structured snippets, images, and promotions
Structured snippets let you list what you offer under a header, for example a Services header followed by the specific treatments you provide. Image assets add a real photo beside your ad, and a genuine shot of your office or team beats a stock graphic every time. Promotion and price assets are worth a look if you run cash based or cosmetic services and want to show a real offer. None of these are mandatory, but each one adds size and specificity, and size and specificity are what get the click.
Do this in the next five minutes
Open an incognito or private browser window and search the exact term a patient would use to find your main service in your town. Look at the ads. Count the links, buttons, and lines under each one. If a competitor's ad is a full block and yours is two lines, you just found free clicks sitting on the table. Now check whether your ad even shows a Book link and a phone button. Most practice owners are surprised by how bare their own ad looks.
A quick word on letting Google do it for you
Google will happily add assets automatically, and it nudges you to let it. Some of that is fine. But automatic sitelinks and callouts sometimes point at the wrong page or use a phrase that reads oddly for a medical practice, and in healthcare the wrong wording is not just sloppy, it can wander into claims you do not want to make. Build your own core assets so every link goes to a real page and every line is accurate and compliant. Let Google's automation be a suggestion you review, never a thing you switch on and forget.
