If you have ever paid for local SEO, you know the map pack. It is the little map with three businesses pinned to it that Google shows near the top when someone searches something like "dermatologist near me." For years, that spot was the prize. Land in those three and the phone rang. Miss it and you were buried below the fold where almost nobody scrolled. Whole marketing budgets were built around one goal: get into the three pack.
So here is a question worth sitting with. When a patient in your town searches for a doctor today, what do they actually look at first? New research suggests the answer has shifted, and if you are still spending like it is 2020, you may be optimizing for a spot patients are starting to skip.
The number that should stop you
In April 2026, rater8 surveyed 992 US adults for its 2026 Patient Choice Report. One question asked which part of a Google results page they trust the most. The answers reorder everything practices thought they knew about local search:
- AI Overview: 37% trust it most
- Organic blue links: 20%
- The local map pack: 13%
- Sponsored ads: 7%
Read that again. The map pack, the thing agencies have charged practices for years to chase, came in at 13%. The AI Overview, that block of text Google now writes at the very top of many searches, pulled 37%. Patients trust the AI answer almost three times more than the three businesses on the map.
Why the map pack lost ground
None of this is because patients suddenly dislike maps. It is because Google changed what the top of the page looks like. The AI Overview now sits above the map pack on a huge share of searches, and it reads like an answer, not an ad. A busy person scanning their phone sees a paragraph that appears to have already done the comparing for them, and they treat it like a recommendation from a friend who did the homework.
The map pack still loads. It is just lower now, and it competes with a block that got there first and looks more helpful. When you have thirty seconds between patients or you are half asleep at 10pm, you take the answer that is right there. That is human, and it is exactly what the data is picking up.
We dug into the trust side of this shift in a separate piece on whether patients trust Google's AI Overview more than your reviews, and the short version is: more than you would like. The AI answer has become the new front door, and a lot of practices do not even know it is there.
Before you rip out your Google Business Profile, read this
Here is where a lot of owners overcorrect. They see a stat like 13% and think the map pack is a waste now. It is not. That would be the wrong lesson, and an expensive one.
The map pack still sends real, ready to book patients. Thirteen percent trusting it most does not mean only 13% use it. Plenty of people scroll to the map to check distance, hours and star rating before they call. More important, the same thing that wins the map pack also feeds the AI answer. Your Google Business Profile, your reviews, your correct hours and services, your photos: that is the clean, verified source Google's AI reaches for when it decides which practice to mention. A strong profile is not the old strategy. It is the foundation of the new one.
So the move is not "abandon the map pack." It is "stop treating it as the finish line." If you want the tactical checklist for the profile itself, we still stand by our guide on how to rank in the Google map pack. Just know that ranking there is now step one of a bigger job, not the whole job.
The new job: be the practice the AI recommends
If patients trust the answer at the top, the real question becomes: how does a practice end up inside that answer? And unlike ads, there is no button to buy your way in. You earn it, roughly the way you earn good rankings, by being the clearest and most trustworthy option the model can find.
In practice, for a medical office, that comes down to a few unglamorous things:
- A website that plainly says who you are. What you treat, where you are, what it costs, who the providers are, in plain language a machine can read. Clever hero taglines do not help an AI understand you. Clear facts do.
- Consistent details everywhere. Your name, address, phone and hours matching across your site, your Google profile and the directories. Conflicting info makes both Google and its AI trust you less.
- Real reviews, kept fresh. Volume and recency signal a real, active practice. The AI leans on that reputation just like a patient does.
- Pages that answer real questions. The exact things patients ask, do you take my insurance, how much is a visit, do you see kids, answered clearly on your site. That is the raw material an AI summary is built from.
This is the whole idea behind what people are now calling generative engine optimization, and we broke down where it is real and where it is hype in SEO and AI search for healthcare in 2026. The reassuring part: there is no secret trick. The practice that is genuinely the clearest, fastest and most trusted in its area is the one the AI keeps naming. Good healthcare SEO and AI visibility are the same discipline pointed at a taller target.
Our honest take
The map pack is not dead, and anyone telling you to dump your Google Business Profile is reading one number and missing the point. Keep it sharp. It still wins clicks and it powers the AI answer above it.
But if your entire local strategy is "rank in the three pack," you are defending a spot patients increasingly glance past on their way to the answer at the top. The practices that win the next few years are the ones that are so clearly described, well reviewed and easy to understand that both Google's map and Google's AI point to them. One is fed by the other. The report even found that 55% of patients have walked away from a doctor based on what they found online, up 15 points in a single year. Attention is moving up the page and getting more ruthless. Your job is to be the practice that still gets picked wherever they happen to look.
