Every year around mid November, we get a version of the same message from a practice owner. Things are getting busy, the holidays are coming, and they are thinking about pausing the ads for a few weeks to save some money. It sounds reasonable. Nobody wants to burn budget on a slow office while half the staff is out for the holidays.

Then January hits, the phones are slow, and they wonder why it takes weeks to get the momentum back. The pause was not free. It just moved the cost somewhere they were not looking. Here is why the holidays are one of the worst times to go dark, and what to do instead if money really is tight.

December patients are not casual, they are on a deadline

The picture in most owners' heads is that nobody is thinking about their health in December. Everyone is shopping, traveling, eating too much. Why would they book a doctor?

Because of money. Three separate financial clocks all run out on December 31, and every one of them pushes a patient to act. Flexible spending accounts work on a use it or lose it rule, and according to Money's reporting, American workers give up roughly 3 to 4 billion dollars in unspent FSA funds every single year because they miss the deadline, an average of over 400 dollars per account. Dental and vision benefits reset to zero on January 1 and do not roll over. And patients who have already met their annual deductible can get care for a fraction of the cost before it resets, which is why elective procedures reliably spike in December.

So the person searching for your practice at 9pm on December 18 is not idly curious. They just realized they have benefits about to vanish, and they are trying not to waste their own money. That is some of the highest intent traffic you will see all year. Pausing your ads means you are invisible to them, and they book with whoever showed up instead. Pausing throws away everything your ads learned

This is the part most owners never hear, and it is the strongest reason not to pause. Modern ad platforms run on machine learning. When you launch a campaign, Google and Meta spend the first stretch figuring out who converts, when, and for how much. That period is called the learning phase, and getting through it is what makes your ads efficient.

When you pause, you throw that progress away. Google's own guidance and the ad community agree that re-enabling a paused campaign is treated like a brand new strategy, so it restarts learning. On Meta it is just as blunt: a pause of seven days or more reliably resets the learning phase, and every ad set then has to gather roughly 50 fresh conversions before it stabilizes again.

Play that out. You pause on December 15 to save a few hundred dollars. You turn the ads back on January 2, when demand is roaring back. But now your campaigns are relearning from zero, so for a week or two they run worse and cost more per lead, exactly when you most want them firing. The holiday you tried to save on just made January more expensive too. The savings were an illusion.

December ads are often cheaper for you, not pricier

There is a myth that ads cost more in December, so you should sit it out. That is true for retail. Ecommerce and shopping keywords do spike hard around Black Friday and Cyber Monday, with some retail cost per click jumping 25 to 35 percent as brands pile into the auction.

But you are not bidding on holiday shoppers. Healthcare search terms live in a different auction entirely, one the gift-buying frenzy never touches. And here is the upside nobody talks about: because so many local practices pause their ads for the holidays out of the same instinct you are fighting right now, there is often less competition for the patients still searching. The clinics that stay on can pick up high intent clicks at a discount while their competitors are dark. You do not want to be the one who left.

Jan is the busiest month of the year for most medical practices, as deductibles reset and new year health resolutions kick in. The ads that carry you into it should already be warmed up.

January is the busiest month, and it starts in December

The other thing a December pause ignores is what comes right after it. For most practices, January is the single busiest month of the year. Deductibles reset, so patients who were waiting feel free to book. And the new year resolution effect is real and measurable: gym memberships jump about 30 percent in January, with roughly one in eight of all annual sign ups happening that month. That same fresh start energy sends people to therapists, weight loss clinics, dentists, and primary care.

People do not flip that switch at midnight on January 1. They start searching and comparing in late December, deciding where they will go. So what should you actually do?

None of this means blindly dumping the same budget into the same generic ads all month. Smart beats loud. Here is the approach we use with the practices we run patient acquisition for.

Lower the budget, do not pause. If cash flow is the worry, cut your daily spend rather than switching campaigns off. A campaign that keeps running on a smaller budget holds onto everything it has learned. A paused one starts over. Same savings, far less damage.

Change the message, not the on switch. Point your ads and landing pages at the year end angle. Use your benefits before they reset, book before December 31, financing available for what insurance does not cover. The click costs the same, but a deadline offer converts far better than a generic one because it matches exactly why the person is searching.

Only pause the truly dead days. If your office is genuinely closed and cannot take or return a call, say December 24 through 26 and January 1, pausing those specific days is fine. You are not throwing away weeks of learning over one long weekend, and you are not paying for clicks nobody can follow up on.

Catch the leads while you are closed. The real risk over the holidays is not wasted spend, it is interest that arrives when nobody is at the desk. Make sure a patient who finds you at 8pm on December 27 can still book. If you need to spend less in December, spend less. Turn the dial down, sharpen the message, and keep a way to catch the leads when the office is closed. Just do not flip the switch off and wonder in February why the machine feels like it is starting from scratch. It is, and that was the expensive part all along.
