First, a gut check on why this matters so much. When someone needs a doctor, they do not open a phone book. They open Google. A widely cited estimate from the National Institutes of Health and others puts health related searches at roughly 7 percent of all of Google's daily traffic, which works out to well over a billion health searches a day. And most of those people never scroll far. If you are not in the first handful of local results, for a patient you basically do not exist.
So let's work through the reasons, starting with the ones that knock out the most practices.
1. Your Google Business Profile is not verified or not claimed
This is the big one, and it catches more practices than you would think. The listing that shows in the map results, the box with your name, reviews, hours, and a call button, is your Google Business Profile. If you have never claimed and verified it, Google is far less likely to show it, and it cannot rank the way a verified one can. Sometimes a listing exists that Google auto generated years ago, sitting there unclaimed, with the wrong hours and no photos.
The fix: claim the profile and finish verification. For healthcare, Google now leans heavily on video verification, where you record a short clip proving the office, signage, and that you have access. It can take a few days to a couple of weeks. Nothing else on this list matters until this is done. We walk through the whole setup in our guide to the Google Business Profile for a medical practice.
2. Your profile is verified but half empty
Claiming the profile is step one. Google ranks the complete ones. A profile with no services listed, three stock looking photos, a blank description, and a category that just says "doctor" gives Google almost nothing to match a patient's search against. Google has said directly that more complete information helps your listing get found for more searches.
The fix: fill in everything. Real photos of the office and team, every service you offer written out, accurate hours including holidays, insurances if you list them, and attributes like wheelchair access or online booking. Then keep it fresh with a post about once a week. We broke down exactly what "complete" means in how a complete Google Business Profile helps a medical practice get found.
3. Your primary category is wrong
Your profile's primary category is one of the single biggest factors deciding which searches you appear for. Pick it wrong and you are invisible for the terms that actually bring patients. A med spa listed as a generic "spa" gets buried under nail salons. A dermatologist listed only as "skin care clinic" misses everyone typing "dermatologist."
The fix: set the most specific primary category that fits what you do, then add secondary categories for your other services. Review it any time you add a service line. More on why this matters in does your Google Business Profile category matter.
4. Your name, address, and phone number do not match across the web
Google cross checks your core details, your name, address, and phone number, against the rest of the internet: your website, Yelp, Healthgrades, your insurance directory listings, old online profiles. When those details disagree, an old suite number here, a tracking phone number there, a slightly different business name somewhere else, Google trusts you less and ranks you lower. In the worst cases the conflicting info splits your reviews and signals across two half listings.
The fix: pick one exact version of your name, address, and phone number and make every listing on the web match it, character for character. This is tedious and it is one of the highest value cleanups in local SEO. We dug into it in do your business listings need to match.
5. You are simply farther away than your competitors
Here is the one most owners miss, because it is invisible from their own desk. Google's local results are built around where the searcher is standing. For "dentist near me," the person two blocks from a competitor will see that competitor first, no matter how good your profile is. Proximity is a ranking factor you cannot fully control.
This is also why your practice shows up when you search from the office and vanishes when a patient searches from across town. You are not being hidden. You are just being measured against whoever is closest to each patient. You can't move your building, but you can win on everything else, category, completeness, reviews, and a strong website, so you still rank for people a reasonable drive away. That is most of what good local SEO actually is.
6. You have too few reviews, or they have gone stale
Reviews feed both ranking and trust. A practice with 12 reviews, newest one from two years ago, loses to the one down the street with a steady stream of fresh ones. And patients notice: research from BrightLocal has repeatedly found that recency matters, with most people trusting only reviews written in roughly the last few weeks. A pile of old reviews reads like a place that used to be good.
The fix: build a simple habit of asking a couple of happy patients a week, by text or email, right after a good visit. Steady beats a one time blast. We put real numbers on it in how many Google reviews a medical practice actually needs.
7. Your website is thin, slow, or missing
Your Business Profile and your website work as a team. The site confirms to Google who you are, carries your services and city in text it can read and quote, and gives found patients a place to book. A practice with no website, or one that loads slowly on a phone, gives Google less to trust and patients less reason to stay. Google has found more than half of mobile visitors leave a page that takes over three seconds to load, so even when you do show up, a slow site loses the patient before they book.
The fix: a fast, clear site with a page for each main service, your location in plain text, and easy booking. More on the speed piece in does website speed affect patient bookings, and if you need the site itself rebuilt, that is what our healthcare websites are for.
8. Your profile was suspended or has a duplicate
If your practice was showing up and then suddenly vanished, this is the likely culprit. Healthcare listings get extra scrutiny, and a profile can be suspended for things that feel small: keyword stuffing the business name, an address change that looks suspicious, a virtual office, or a flagged edit. Duplicate listings, two profiles for the same practice, can also cancel each other out and confuse Google about which one to show.
The fix: open your Business Profile dashboard and look for a suspension notice. If you are suspended, file for reinstatement with proof you are a real practice at that address, a utility bill, signage photos, your license. If you have duplicates, ask Google to merge or remove the extra one. A related nightmare is when someone else edits or takes over your listing, which we covered in what to do if your Google Business Profile gets hijacked.
9. Sometimes Google just does not show local results for that search
Last one, and it is a relief as much as a reason. Not every search triggers the map pack. For some broad or informational queries, Google shows plain blue links or an AI answer and no local box at all. In fact, when BrightLocal studied this, the local pack appeared for only about 15 percent of searches overall, though it is far more common for the "near me" and "specialty plus city" terms patients actually use when they want a doctor.
The fix: make sure you are checking the right searches. Test the exact phrases a patient would type to book, like "pediatrician [your city]" or "urgent care near me," not a broad term like "healthcare." And test them from outside your office, ideally in an incognito window or using your profile's own performance insights, so you see what a real patient sees rather than your personalized results.
How to tell which reason is yours
You do not have to guess. Run this quick self check:
- Search in incognito, away from the office. Use the real phrases patients use. If you appear for some and not others, it is usually category or completeness, not a suspension.
- Open your Business Profile dashboard. Look for a verification prompt, a suspension banner, or missing fields. This catches reasons 1, 2, and 8 fast.
- Google your own name, address, and phone number. See if the details match everywhere. Mismatches are reason 4.
- Check your reviews and your site speed. Few or old reviews, or a slow mobile site, are reasons 6 and 7.
Most practices find two or three of these at once. The good news is that every one of them is fixable, and none of them require you to outspend the hospital system down the road.
Where EtherealMinds fits
We work only with healthcare, so this exact problem is most of what a new client brings us. We start with an audit that runs the checklist above, claim and verify your profile if it is not, clean up every mismatched listing on the web, set the right categories, and get a steady review and posting rhythm going. Then we make sure your website backs it all up. That whole effort, getting you found when a patient searches, is our healthcare SEO work.
But showing up is only half of it. Plenty of practices rank fine and still lose the patient, because the call that the listing earns goes to voicemail, or the site is confusing, or nobody follows up. That is why we wire the profile, the website, the reviews, and the follow up into one patient acquisition system. Being on Google only pays off when the patient it sends you actually gets booked.
The bottom line
If your medical practice is not showing up on Google, it is not bad luck and Google is not out to get you. It is almost always an unverified or half finished profile, the wrong category, mismatched listings, distance, weak reviews, a thin site, or a suspension, usually a few of those together. Work the checklist, fix what you find, and give it a couple of months. Patients are searching for exactly what you do, right now, in your town. The only question is whether they find you or the practice down the street.
