A patient searching for a doctor on a phone, where Google AI now answers before showing the map

Something shifted in how patients find a doctor, and most practice owners have not noticed yet because it happened on the screen, not in the inbox. For years the game was the map pack: rank in the top three local results, show a few reviews and a phone number, and you got the call. There was room. Three practices in the box, ten more a click away, and anyone determined enough could scroll and find you.

Google's AI changed the shape of that page. On a growing share of searches, the first thing a patient sees is not a list at all. It is an answer. An AI written summary that reads the question, reaches into Google Business Profiles, websites, and reviews, and hands back a tidy paragraph naming a couple of practices it thinks fit. The patient reads it like advice from a knowledgeable friend. And here is the uncomfortable part: if your practice is not in that paragraph, for a lot of those people, you no longer exist.

Two features, one shift

It helps to name what is actually happening, because the two pieces get muddled. AI Overviews are the summaries that now appear automatically at the top of an ordinary results page. You did not ask for AI, you just searched, and there it is above everything else. AI Mode is the separate conversational tab, powered by Google's Gemini model, where the entire page becomes a back and forth answer and you can ask follow up questions like "which of those takes my insurance." Google said AI Mode crossed a billion monthly users in 2026. This is not a lab experiment anymore. It is how a huge chunk of the country searches.

For a practice, the distinction matters less than the pattern they share. Both of them answer the search instead of just listing options, and both name only a handful of businesses before they stop. The old page rewarded being in the top ten. The new one rewards being one of the two or three the AI decides to mention. The funnel to your front door got narrower, and it got narrower fast.

35% & 91% More organic clicks (35%) and paid clicks (91%) that a brand earns when it is cited inside a Google AI Overview versus not cited at all, per a September 2025 study by Seer Interactive. Being named is now most of the game.

"But nobody clicks anymore, so why bother?"

This is the objection we hear, and it is half right. Yes, when the AI answers the question, fewer people click through to any website at all. That is real, and we wrote about it in what zero click search means for your practice. But the conclusion people draw from it is wrong. They hear "fewer clicks" and assume the whole thing stopped mattering. The opposite is true. The clicks that remain are concentrating on the few names the AI puts forward.

Seer Interactive's September 2025 analysis, which tracked millions of real search impressions, found that when a brand is cited inside an AI Overview it earns roughly 35 percent more organic clicks and 91 percent more paid clicks than a brand that is not cited. Read that twice. The AI did not kill the click. It decided who gets it. If the machine is going to hand the shortlist to the patient, you want your name on the short list, because the alternative is not a smaller slice of attention. It is none.

So how does Google's AI pick?

There is no secret dial, and anyone selling you a guaranteed spot inside an AI answer is selling smoke. The AI is not inventing a new popularity contest. It is reading the same signals Google already uses to decide who is a credible local option, and summarizing them. That is actually good news, because it means the work is knowable. Four things do most of the lifting.

1. A complete, living Google Business Profile

Your Google Business Profile is the single richest thing the AI reads about you. Hours, services, insurance accepted, photos, categories, the questions and answers, the posts. When it is filled out and active, the AI has plenty of accurate material to pull a confident sentence from. When it is half empty, you have given it nothing to say, so it reaches for the practice down the street that did the work. We broke down the details in why a complete Google Business Profile matters, and it has never mattered more than now.

2. The same facts everywhere

If your address says Suite 200 on Google, Suite 2 on Healthgrades, and nothing on your own contact page, the AI has to guess which is true, and guessing makes it cautious. Machines trust consistency. One spelling of your name, one phone number, one address, repeated cleanly across your website, your profile, and the directories. Mismatched listings are one of the most overlooked reasons a practice gets passed over, and they are completely fixable.

3. Fresh, genuine reviews

Google's AI does not just count your stars, it reads your reviews and often repeats what patients said about you. A clinic with a steady flow of recent, specific reviews looks alive and trusted. A clinic with eleven reviews from 2022 looks like a place that used to be good. Recency is its own signal, which is why the flow matters more than the pile, a point we make in how reviews affect your Google ranking. Keep them coming and keep answering them.

4. A website that answers real questions

The AI loves a website that talks like a patient, not like a chart. Pages that answer the actual things people ask, do you take my insurance, do you see kids, how fast can I get in, what does this cost, give it clean, quotable material. Jargon and brochure fluff give it nothing. This is the same muscle that wins old fashioned search too, so you are not choosing between ranking on Google and showing up in AI. It is one job. We go deeper on it in optimizing for AI search in healthcare.

A reality check worth keeping

The AI answers out of what already exists about you online. It is a mirror, not a magician. If your profile is thin, your listings disagree, and your last review is a year old, no clever prompt or trick will make the machine recommend you. Fix the raw material and the AI starts describing a practice worth choosing, because that is finally what it finds.

The part most owners miss: the AI is not the finish line

Getting named in the answer is step one. The moment a patient reads "Dr. Lee's office is well reviewed for pediatric care," their next move is to check you out, and that still usually means your reviews and your website. The AI opens the door. Your profile and your site decide whether they walk through it. We watched this exact pattern in how patients discover you in AI but book on your site. A great AI mention wired to a slow, confusing website is a handshake followed by a slammed door.

Which is why chasing AI visibility as a separate project is a mistake. The things that get you named, a strong profile, clean listings, fresh reviews, a clear website, are the same things that convert the patient once they arrive. You are not building for the robot. You are building for the patient, and the robot happens to reward the same work.

Where EtherealMinds fits

You can start this yourself today. Open your Google Business Profile and finish it. Search your practice name across the big directories and fix anything that disagrees. Set up a simple habit for asking happy patients for reviews. Go through your website and answer the questions patients actually ask, in plain words. That alone will move you, and we would rather you began than waited.

Where practices get stuck is keeping all of it true and current while running a full schedule. That is the part we own. When we build a practice's healthcare SEO and patient acquisition system, we keep the profile complete, the listings consistent, and the reviews flowing, and we shape the website patients land on so both Google's AI and the human reading it come away with the same clear answer: this is the practice to pick. The goal is not to trick a model. It is to genuinely be the best local option, and to make sure the machine can tell.
