A physical therapy clinic owner sent us a screenshot last month with one line: "Am I doing something wrong?" Two of his keywords sat at a Quality Score of 3. He was paying more than the clinic down the road for a worse position on the exact same search, and he could not figure out why. He assumed the fix was to bid higher. It was the opposite. He needed to earn a better score, and then he could bid less.

Quality Score is one of those things Google puts right in front of you and never really explains. So owners ignore it, keep raising bids to fight for the top spot, and wonder why healthcare clicks feel so brutal. What Quality Score actually is

Quality Score is Google's rating, from 1 to 10, of how relevant and useful your ad and landing page are to the person who searched. It is calculated per keyword, not for the whole account, so you will have strong keywords and weak ones sitting side by side. Google builds every score from three ingredients, and each one is graded in your account as below average, average or above average:

When all three are above average, your Quality Score climbs toward 10. When they sag, it drops toward 1. That single number then feeds into your Ad Rank, which is roughly your bid multiplied by your quality. This is the part that stings: a competitor with a better Quality Score can outrank you while bidding less than you. Their relevance is doing the work your money is trying to do.

So what counts as a good score?

For your most valuable, highest intent keywords, the ones where a click can become a surgery or a long term patient, aim for 8 or higher. If the keyword is dental implants in Austin, the headline should say dental implants, not Trusted Family Dentistry. Mirror the search, name the service, name the city, and put one clear promise in front of them. Tight ads that echo the search win the relevance grade, and Google rewards that grade with a cheaper click. Vague, one size fits all ads are taxing you on every impression.

3. Fix your landing page experience

This is the lever practices ignore most and lose the most on. Most run an ad for one specific service and then send the click to their general homepage, where the visitor has to hunt for what they wanted. Google watches them bounce and marks the page down. Every ad should point to a focused landing page about that one service, with the search terms on the page and one obvious way to book. 1 to 10 Quality Score is set per keyword, not per account. That means you can lift the handful of keywords that actually book patients without perfecting every term in the account. Focus the work where the money is.

The leak Quality Score will never fix

Here is where we get honest, because a great score can still lose you every patient. Quality Score buys you a cheaper, higher click. It does not answer your phone. You can win the auction, land the visitor on a beautiful page, and still lose them if the booking form is broken or, far more often, if they call and reach voicemail.
