A family practice owner in Ohio called us in August with a puzzle. Her Google views were up almost a third over the spring. Her phone was ringing about the same as always. "So which number do I believe," she asked, "the one that says more people are seeing us, or the one on my appointment book?"

Both, as it turns out. More people really were seeing her practice. They just were not clicking the way they used to, because they did not need to. A growing share of them were reading everything they wanted, her hours, her rating, the services she lists, inside an answer Google or an AI assistant handed them, without ever tapping through to the website or the call button.

That is the shift under way right now, and it changes how you should read your own numbers. Let me walk through what actually happened to the Google Business Profile in 2026, what the data says about how patients search today, and the practical moves that keep your practice the one that gets named.

What changed on your listing in 2026

For years, a Google Business Profile was simple to think about. Someone searched "dermatologist near me," they saw a little map with three practices, they picked one and clicked. You could watch the calls and clicks add up and know roughly where they came from.

Two things broke that clean picture this year.

Ask Maps arrived. In March 2026, Google rolled out a Gemini powered feature inside Maps that answers questions in plain conversation. Ask it "which nearby clinic can see my kid for an ear infection today," and it writes back an answer built from business profiles, reviews and websites, your listing among them. The old questions and answers box is gone. In its place is an assistant that reads your profile and speaks for you.

AI Overviews took over the top of the page. The block of AI text that now sits above the normal search results pulls in local businesses too. So a patient searching for care often gets a written summary, sometimes naming a practice or two, before they ever reach the map or the links.

Google also updated the Performance report inside your profile to reflect this. Reports through 2026 began separating how people reach you by surface, splitting the classic local result from the newer AI powered views. The single "views" number you used to trust is now several different doors, and more of them open inside an AI answer than a year ago.

Nearly half of patients now search with AI

This is not a handful of early adopters. The 2026 Patient Choice Report from rater8, which surveyed close to a thousand US adults, found that 47% of patients said they used AI to research a provider by the middle of 2026, up from 31% at the end of 2025. That is a jump of roughly half in about nine months.

Reporting on the same study, TechTarget noted that among people who actively looked for a doctor in the past year, AI tools like ChatGPT were cited as an influence by 36%, edging past plain Google search. And the age group leaning on AI the hardest was not the youngest. It was patients between 45 and 60, the ones with the appointments and the insurance and the money.

Put those two facts side by side. Your listing is being read by machines, and nearly half of your future patients are asking those machines who to see. The place where you win or lose a patient has moved from a click to a sentence in an answer box.

How this loses you patients you never see

Here is the part that stings. When an AI answers a patient's question for them, that patient often decides right there and never lands on your site, so you never see the visit in your analytics. That is fine when the answer is accurate and flattering. It is a real problem when it is not. In the same rater8 survey, two thirds of patients who used AI to research a provider said they had run into incorrect provider information, and 60% said they trusted the AI summary without checking it. So if your profile lists Tuesday hours you changed a year ago, or a phone number from the old office, the AI may repeat it with total confidence, and most patients will believe it. You lose the booking and never learn why.

What actually feeds the AI answer

The good news is that these tools are not magic and they are not random. They read specific sources, and you control most of them. Three feed almost every answer about a local practice.

Your Google Business Profile. This is the backbone. Hours, categories, services, address, phone, the photos, the description. A complete, correct, recently updated profile gives the AI clean material to quote. A half empty one gives it reasons to skip you. Your reviews. Ask Maps and the AI summaries lean heavily on what patients wrote. Recent reviews that mention the specific things you do, "she got my son in same day for an ear infection," are gold, because they match the exact questions patients ask the AI. A steady flow of fresh reviews does double duty now: it lifts your ranking and it feeds the sentence the AI writes about you.

Your website. This is where the deeper facts live. Which insurance you take, the conditions you treat, what a first visit costs, how to prepare. When your pages answer those plainly, the AI has something to pull. When your site is thin, three pages and a phone number, there is nothing for it to grab, and it reaches for a bigger competitor instead. How to check where you stand this week

You do not need a consultant to get a first read. Two quick things.

Read your Performance report as a trend, not a single number. Open the report inside your Google Business Profile and watch calls, direction requests and website clicks against total views over a few months. If views keep climbing while the actions stay flat, that is your sign that more patients are getting their answer inside an AI surface and deciding before they click. That is not a failure. It just means the answer itself, not your website, is doing the talking, so the answer had better be right.

Ask the machines about yourself. Open ChatGPT and Google and ask each to recommend a practice like yours in your city. Do you get named? Are the details correct? Try it the way a nervous patient would type it, in full sentences, at 10pm. What comes back is roughly what your future patients are seeing, and it tells you fast whether you are in the conversation or invisible.
