A pain management clinic called us convinced their main competitor had some secret budget or agency magic they could never match. The competitor was showing up in the feed, ranking near the top of the map, and pulling in reviews. So we spent twenty minutes doing something the clinic did not know was possible: we pulled up every single ad the competitor was running, read their last hundred reviews, and studied their Google listing. No secret budget. They ran three simple video ads, all about one service, and their reviews kept praising the same easy booking flow. That was the whole trick. It was all sitting in public, and once we could see it, the mystery was gone and the plan wrote itself.
Most practice owners think competitor research means paying for a fancy tool or hiring a spy. It does not. The best sources are free, public, and built by Google and Meta on purpose. Let us walk through exactly where to look, what to read, and the one mistake that makes all of it useless.
Start with the ad libraries, because ads never lie about priorities
A business will tell you anything on its homepage. But where a business spends real ad money tells you what it actually believes brings in patients. Two public tools show you that spend, and neither costs a cent.
The Meta Ad Library: every Facebook and Instagram ad, in the open
Meta runs a free public database of every active ad on Facebook and Instagram. It lives at facebook.com/ads/library, and you do not need an account or a login to use it. Set the country to United States, pick the All ads category, and type in a competitor's practice name. Up pops every ad they are running right now: the image or video, the exact words, and the date it started.
That start date is the gold. An ad that has been running for four months is an ad that works, because no owner keeps paying for a loser. So when you see one creative that has been live since spring while five others came and went, you are looking at the message that converts for them. You just learned, for free, what took them months and real dollars to find out. Meta built this for political transparency, but it turned into the most useful competitor research tool marketers have, and healthcare is no exception.
Google's Ads Transparency Center: their Search and YouTube ads
Google has its own version. The Ads Transparency Center launched in 2023 and lets anyone search an advertiser or their website and see the ads they run across Google Search, YouTube, Display, and Gmail. You get the creative, the format, and the last date each ad ran.
Two honest limits, so you are not surprised. It does not reveal the exact keywords a competitor bids on, and it shows zero performance data. So you learn what they say and how active they are, not how well any of it works. Still, seeing that a rival runs a steady stream of search ads for one specific procedure tells you where they are putting their chips. Read their reviews like a focus group they paid for
A competitor's reviews are the most honest market research in your town, and their patients wrote it for you. Open their Google Business Profile and do two passes.
First, read the five star reviews and look for the words that repeat. If patient after patient praises the short wait, the friendly front desk, or how easy it was to book online, that is the promise winning them patients. Second, and this is where the real opening hides, read every one and two star review. The complaints are a map of the gaps you can fill. If their patients keep grumbling about rushed visits, long holds, or a phone that goes to voicemail, you now know exactly what to be better at and what to say on your own site.
The gap hunt
Make two columns while you read a competitor's reviews. Left column: what patients love, so you know the bar you have to clear. Right column: what patients complain about, because every complaint is a promise you can make and keep. A rival with 400 reviews and a wall of "could never get anyone on the phone" is handing you your whole marketing angle for free.

Study their Google listing and website like a patient would
Now look at the rest of their public footprint the way a nervous new patient does at 9pm on their phone.
- Their Google Business Profile category. The primary category is one of the biggest reasons a listing shows up on the map or does not. If a med spa competitor is winning the map, check whether they are listed as "Medical spa" while you are stuck under "Skin care clinic." That one dropdown moves rankings.
- Their photos and post activity. Are they posting fresh photos and updates, or is the listing frozen in 2022? A stale listing is a soft competitor.
- The path from click to booked. Click their "Book" button. Count how many taps it takes to actually get an appointment. If it is fast and clean, that is part of why they win. If it is clunky, that is your edge.
- What services they lead with. The service on their homepage and in their ads is usually their most profitable one. It also tells you what the local market is hungry for.

Mystery shop them, gently
Numbers and screenshots only take you so far. The last piece is the human one. Call the competitor as a normal person with a normal question. How fast did someone pick up? Were they warm or rushed? Did they offer you an appointment or just take a message? Fill out their web form and time how long until anyone follows up. You will learn more about why they win or lose patients in one phone call than in an hour of scrolling.
Then, and this is the uncomfortable part, do the exact same thing to your own practice. Call your front desk as a stranger. Fill out your own form. Most owners are shocked at the gap between what they think happens and what a real patient gets. The one mistake that ruins all of this
Here is the trap. Owners do a little competitor research, see a rival's ad, and immediately copy it word for word. Now they are running a slightly worse version of someone else's message, and patients can smell a me too practice from a mile away. Copying makes you the runner up by design.
Research is for finding patterns and gaps, not lines to steal. Use it to answer better questions. What service does everyone in town advertise, so you might win by owning the one nobody talks about? What complaint shows up in every competitor's reviews, so you can build your whole promise around fixing it? Where is a competitor absent, a nearby town they ignore, a language they never speak, an evening hour nobody offers? The goal of watching everyone else is to figure out how to be different on purpose, not how to blend in.
A quick weekend audit
Pick your top three competitors. For each one, spend fifteen minutes: pull their ads in both libraries, skim their reviews for one thing patients love and one thing they hate, click their booking button, and make one phone call. Write it all on a single page. By Sunday night you will understand your local market better than you have in years, and you will not have spent a dollar.
