Two runners neck and neck on a track, a picture of a medical practice competing with a rival for the top spot on Google
Ranking on Google is a race between you and the two or three practices nearest you. You do not have to beat the whole city, just the runners in your lane. Photo by RUN 4 FFWPU via Pexels.

A physical therapist called us in the spring, genuinely stung. He had twenty years of experience, a wall of certifications, and patients who loved him. Two miles away, a newer clinic with a fraction of his training was sitting in the top spot on Google Maps for every search that mattered. Every time he searched, there they were above him. He wanted to know one thing: how is that possible when I am clearly the better practice?

Here is the answer nobody likes to hear. Google is not judging who the better doctor is. It cannot. It has no way to measure bedside manner or surgical hands or how many lives you have changed. So it ranks the things it can actually see. Once you understand what those things are, the mystery disappears, and so does most of the frustration, because most of it is fixable.

Google looks at three things, not your resume

Google spells this out in its own local ranking documentation. When it decides who shows up in the map results, it weighs three factors: relevance, distance, and prominence. That is it. Your competitor is not beating you on medicine. They are beating you on one or more of these three, and usually they do not even know it. They just happened to set things up in a way Google rewards.

Let me walk through each one, because the fix lives inside them.

1. Relevance: does your profile match what the patient typed

Relevance is how well your Google Business Profile matches the actual search. A patient types dermatologist near me, or med spa, or physical therapy for back pain. Google looks at your profile and asks, does this business clearly do that thing. The biggest lever here is your primary category, and this is where so many practices lose without ever noticing.

In Whitespark's 2026 Local Search Ranking Factors report, the primary Google Business Profile category is the number one ranking factor overall. Not reviews. Not your website. The category dropdown. If your competitor set theirs to Medical Spa and you set yours to the vaguer Skin Care Clinic, they win searches for med spa before a single review is counted. Most owners pick a category once when they claim the listing and never think about it again. It might be the most important dropdown in your whole marketing, and it takes two minutes to check.

2. Distance: how close you are to the person searching

Distance is exactly what it sounds like. Google ranks results partly by how close your office is to wherever the patient is standing when they search. This is the factor you control the least, and it explains a trap almost every owner falls into.

When you search your own practice from inside your office, you sit right at the top. It feels great and it lies to you. Google knows where you are, and you are zero miles from your own address, so of course you rank first. Open an incognito window and search from home, or from the other side of town, and you will see the real picture, the one your patients see. If a competitor is more central to your patient base, they get a distance edge you cannot fully erase. What you can do is make the other two factors so strong that Google shows you anyway, even when you are not the closest option.

3. Prominence: how known and trusted you look online

Prominence is Google's read on how well known and reputable your practice is. It pulls this from your reviews, your links and mentions around the web, your directory listings, and the strength of your website. As BrightLocal notes in its breakdown of the local algorithm, reviews are one of the strongest prominence signals Google uses, both the number of them and how positive they are.

This is usually the real gap. The competitor outranking you is often the one collecting a handful of fresh reviews every week while your review count froze two years ago. Whitespark's 2026 data shows review signals carrying even more weight than before, and Google looks at recency, not just the total. Fifty great reviews that all stopped in 2024 send a weaker signal than fifteen reviews that keep arriving this month.

So why does a worse practice win

Put the three together and the picture is clear. Your competitor is winning because their profile is more complete, their primary category is a sharper match, they are close enough to the searcher, and their reviews are more recent and more numerous. None of that makes them a better clinician. It makes them a better match for what Google can measure. We wrote about the review side of this fight in detail in how to compete with a competitor who has more reviews, because it is the single most common gap we see.

The usual reasons a competitor outranks you

1. Their primary category is a better match and yours is vague or wrong. 2. Their profile is fully filled out, services, photos, hours, description, and yours is half blank. 3. They collect reviews every week while yours went silent. 4. Their name, address, and phone match everywhere online and yours conflict across old directories. 5. Their website is fast and clearly local, with your city and service on the page, while yours is slow or generic. 6. They are simply closer to more of the patients searching. Five of those six are things you can fix.

How to close the gap, step by step

You do not have to outrank the entire city. You have to beat the two or three practices sitting next to you in the local map results. That is a much smaller race than it feels like at 9pm when you are staring at a competitor's listing. Here is the order we work in.

Fix your primary category first

Open your Google Business Profile and look at your primary category with fresh eyes. Is it the exact phrase a patient would search for your main service? If not, change it. Then add every relevant secondary category for your other services. This is the highest return two minutes in local SEO, and it is free.

Fill out every field, then add photos

A complete profile beats a thin one. Add your services, your hours, a real description, your website, and a steady stream of photos of your actual space and team. Google rewards complete profiles with more visibility, and patients trust them more. A half empty listing tells Google you are not paying attention, and it ranks you like it.

Turn reviews back on and keep them flowing

Build a simple habit of asking every happy patient for a review, and make it one tap by sending them your direct Google review link. Aim for a few new ones every week, not a burst once a year. Reply to each one, good or bad. This is the factor that moves prominence the most over time, and it is why we treat it as a weekly system, not a favor you ask when things get slow. The mechanics of why reviews move rankings are in whether reviews affect Google ranking.

Make your name, address, and phone identical everywhere

Google cross checks your details across Yelp, Healthgrades, your old Facebook page, and dozens of directories. When they disagree, Google trusts you less and ranks you lower. Getting them all to match is tedious and it works. We broke down that cleanup in whether your practice info matches everywhere online.

Give Google a fast, clearly local website to trust

Your website feeds prominence and relevance both. It should load fast on a phone, name your city and your services in plain text, and give patients an obvious way to book. A slow, generic site drags your ranking and loses the patient once they finally click through. If yours is holding you back, that is exactly the kind of thing our websites that convert and rank are built to fix.

Our honest take: it is a race you can win

The thing that stings about ranking below a lesser practice is that it feels unfair, and in a way it is. Google is grading a test that has nothing to do with how good a doctor you are. But here is the reframe we give every owner: that same unfairness is your opening. The competitor beating you is not doing anything you cannot do. They filled out a free profile, picked a smart category, and asked for reviews. You can out execute that, and your actual medicine being better means you will collect those reviews faster once patients are through the door.

The practices that win local search are almost never the ones with the biggest budgets. They are the ones who treat their Google profile like a living part of the business instead of a form they filled out once. Steady beats flashy here. A few reviews a week, an accurate profile, and a fast website will pass a competitor who set everything up two years ago and walked away.

How EtherealMinds helps you take the top spot

When we run local SEO for a practice, we start exactly where this article does. We audit your Google Business Profile against the competitors actually outranking you, fix the category and the gaps, clean up your listings across the web so your details match everywhere, and build a review system that keeps fresh five star feedback coming in every week. Then we tie it into the rest of your patient acquisition system, so the patients who finally find you at the top can book in one tap and reach a real person or an AI receptionist when they call. If you have ever wondered why you are not showing up at all, not just below a rival, start with why your practice is not showing up on Google.

So, why is your competitor outranking you on Google? Not because they are better at medicine. Because they are winning relevance, distance, and prominence, and two of those three are sitting right there for you to take. Go search your own practice in an incognito window from across town, look at who is above you, and check their category and their recent reviews against yours. You just found your to do list.
